Before: The patient is 5 feet 4 inches tall, weighs 136 pounds, and has a body mass index (BMI) of 20. He presents with relatively small breasts, with the right breast exhibiting significant ptosis. The patient wishes to preserve nipple sensation if possible.
Given these factors, the recommended approach is a buttonhole technique. The patient’s size allows the procedure to be performed using an inferior pedicle, which supports nipple sensation preservation.
A double-incision procedure will be necessary on the right side to correct the ptosis. To achieve optimal symmetry, a double incision will also be performed on the left side. The surgical plan includes tailor-tacking the skin to ensure precise symmetry before creating the pedicle for the buttonhole procedure.
After: The patient presents with a well-defined, masculine chest contour. Surgical incisions have healed exceptionally well and are nearly imperceptible. The nipples remain pink with intact sensation, and there are no dog-ear deformities bilaterally.
The right side of the chest wall appears more prominent; this is attributed to a bony ribcage variation rather than soft tissue. A total of 115 grams of tissue was removed from the left side and 105 grams from the right side, resulting in a balanced and natural chest appearance.
Jordan, Buttonhole chest reconstruction surgery
Before: The patient is 5 feet 7 inches tall, weighs 168.4 pounds, and has a body mass index (BMI) of 26.3. He presents with small breasts and a short distance between the inframammary fold (IMF) and the nipple-areolar complex (NAC). The NAC is relatively small and already exhibits a masculine appearance.
Additional findings include fat pads in the axillary and sternal regions, which may be addressed during the surgical procedure to enhance chest contour.
After: The buttonhole pedicle does not significantly alter the breast appearance. Due to a small amount of additional abdominal fat, there is no noticeable feminine appearance to the nipple-areolar complexes (NACs). The NACs are appropriately sized for a masculine chest, and nipple sensation is returning on the right side.
A total of 445 grams of tissue was removed from the left side and 420 grams from the right side, resulting in a balanced and natural chest contour.
Eli, Buttonhole chest reconstruction surgery
Before: The patient is 5 feet 8 inches tall, weighs 158 pounds, and has a body mass index (BMI) of 24. On examination, the patient appears healthy and presents with a moderately sized chest.
The patient’s goal is to achieve a flat chest contour. Examination suggests that a double-incision approach can be performed without crossing the midline. The patient expresses a preference to preserve nipple sensation if possible. Given the amount of breast tissue present, a buttonhole technique would be feasible; if not, a free nipple graft may be considered to achieve the desired result.
After: The patient presents with a well-defined, masculine chest contour. Surgical incisions demonstrate excellent healing, appearing fully healed beyond three weeks postoperatively. The nipples are pink with intact sensation, and there are no dog-ear deformities bilaterally.
A total of 330 grams of tissue was removed from the left side and 470 grams from the right side, resulting in a balanced and natural chest appearance.
Alysia, Buttonhole chest reconstruction surgery
Before: The patient is 5 feet 5 inches tall, weighs 190.6 pounds, and has a body mass index (BMI) of 31.7. He presents with small breasts without ptosis and demonstrates good skin turgor.
The chest exhibits a very masculine contour with prominent pectoralis muscles and inferior fullness above the inframammary fold (IMF). The nipples are positioned just inferior to the ideal masculine location but maintain an appropriate distance from the sternum to preserve a masculine appearance.
Additional observations include minimal adiposity, masculine-appearing areolae, and nipple inversion.
After: The patient presents with a well-defined, masculine chest contour. Surgical incisions are clean, dry, and intact (C/D/I) with no signs of infection (SOI). The scars are slightly raised but remain minimal. The nipples retain sensation, and there are no dog-ear deformities bilaterally.
A total of 210 grams of tissue was removed from the left side and 145 grams from the right side, resulting in a balanced and natural chest appearance.
