Before: The patient is five feet seven inches and weighs 286.2 pounds, with a BMI of 44.8. On examination, the patient has large, empty, pendulous breasts with significant loss of upper pole fullness and minimal volume in the upper breast. The left lateral roll is more prominent compared to the right. The breasts are closely positioned along the midline and may have a tendency to overlap. Additional chest and flank adiposity is noted, contributing to overall breast and chest contour considerations.
After: On examination, the patient demonstrates a masculine chest contour with significant improvement compared to the preoperative appearance. Mild asymmetry remains; however, the overall differences in contour between the two sides are subtle and within an expected range.
A total of 7905 grams of tissue was removed from the left side and 8325 grams of tissue was removed from the right side.

Before: The patient is five feet eight inches tall and weighs 165 pounds, with a BMI of 25.1. On physical examination, the patient has moderate-sized breasts with grade II ptosis. There is also the presence of lateral chest fullness/rolls and excess skin related to prior weight loss. These findings were considered during surgical planning to achieve an improved breast contour and overall chest appearance.
After: Postoperative examination revealed that the incisions are clean, dry, and intact with appropriate healing. There is no evidence of erythema, induration, hematoma, keloid formation, or significant incisional thickening. A total of 795 grams of tissue was removed from the left side and 750 grams from the right side during the procedure.

Before: The patient is 5 feet 5 inches tall, weighs 189 pounds, and has a body mass index (BMI) of 31.5. On examination, he presents with fairly large breasts that extend toward the midline, making it likely that the surgical incision will cross the midline to achieve optimal contour and symmetry.
Lateral chest rolls are also present and will be addressed during the procedure to enhance the overall masculine chest contour.
After: On examination, the incisions have healed exceptionally well, with thin, flat scars. The nipple grafts have taken successfully, displaying even pigmentation. There are no dog-ear deformities bilaterally.
A total of 1,360 grams of tissue was removed from the left side and 1,125 grams from the right side, resulting in a balanced and natural chest contour.

Before: The patient is five feet seven inches tall and weighs 274 pounds, with a BMI of 42. On examination, there is moderate breast tissue with Grade II ptosis. The patient demonstrates good muscular definition and adequate skin turgor with sufficient elasticity to accommodate a double-incision approach. Mild lateral chest fullness is present. Due to the anatomy, the surgical incision may potentially extend across the midline if required to achieve optimal contouring.
After: On examination, the chest demonstrates a well-contoured appearance. The incisions appear well-healed with no signs of infection. There is no evidence of dog-ear formation bilaterally. The nipple grafts have taken well with appropriate healing. A total of 1,585 grams of tissue was removed from the left side and 1,405 grams from the right side.

Before: The patient is five feet one inch tall and weighs 179.4 pounds, with a BMI of 33.8. Physical examination demonstrates significant breast tissue with bilateral Grade II ptosis (breast sagging). Due to the close spacing of the breasts, the surgical incision may need to extend slightly across the midline to achieve optimal contouring. The patient also has notable lateral chest fullness and excess tissue, which will be addressed during surgery. Additional areas of fullness, including the axillary, sternal, and lateral chest fat pads, will be treated with contouring techniques to create a smoother, more masculine chest appearance.
After: On examination, the chest demonstrates a masculine, well-contoured appearance. The surgical incisions have healed well with no evidence of dog-ear formation. The nipple grafts have healed appropriately with satisfactory overall appearance. Mild residual posterior chest fullness/rolls remain. During surgery, a total of 3.8 kilograms of tissue was removed from the left side and 4.7 kilograms from the right side, with an additional 1,500 milliliters of fat removed through liposuction to enhance chest contour and definition.

Before: The patient is five feet five inches tall and weighs 196 pounds, with a BMI of 33.1. On examination, there is moderate breast tissue with bilateral Grade III ptosis. Prominent lateral chest rolls are also present. Accessory nipples are noted bilaterally, located approximately 2–3 fingerbreadths below the inframammary fold.
After: On examination, the incisions are thin, flat, and well-healed without evidence of hypertrophy. There is no erythema, induration, or hematoma formation. Mild residual swelling is present. A total of 825 grams of tissue was removed from the left side and 820 grams from the right side.

Before: The patient is five feet three inches tall and weighs 248 pounds, with a BMI of 43.9. Physical examination demonstrates significant breast development with prominent lateral chest fullness. Based on the patient’s anatomy and goals, a double-incision mastectomy with free nipple grafts and excision of the lateral chest rolls is recommended to achieve a flatter, more contoured chest appearance.
After: On examination, the chest demonstrates a masculine, contoured appearance with well-healed surgical incisions. There is no evidence of dog-ear formation, and the nipple grafts have healed appropriately with good overall results. Mild residual posterior fullness/rolls are present. During surgery, a total of 3.8 kilograms of tissue was removed from the left side and 4.7 kilograms from the right side, with an additional 1,500 milliliters of fat removed through liposuction to further refine and contour the chest.
