Before: The patient is 5 feet 4 inches tall and weighs 190 pounds, resulting in a body mass index (BMI) of 32.5. He presents with moderately large breasts and bilateral Grade 2 ptosis. A double-incision mastectomy with free nipple grafts is recommended to achieve the desired chest contour.
There is no significant lateral chest roll; however, axillary and sternal fat pads are present and will be addressed during the procedure to enhance the overall masculine chest appearance. The patient has also expressed a preference for masculine-appearing nipples.

After: On examination, the patient demonstrates a well-defined, masculine chest contour. The incisions have healed beautifully and are nearly undetectable beneath the chest hair. Nipple grafts show excellent healing and uniform pigmentation. There are no dog-ear deformities bilaterally.
A total of 1,160 grams of tissue was removed from the left side and 1,140 grams from the right side, achieving a balanced and symmetrical result.

Before: The patient is 5 feet 7 inches tall and weighs 193 pounds, with a body mass index (BMI) of 30.2. On examination, there is Grade II breast ptosis. The right lower thoracic ribs are more pronounced, and the left breast is slightly larger in comparison.

After: The patient maintains nipple sensation, with nipples and incisions appearing symmetrical. There is no evidence of keloid formation or scar thickening.
A total of 1,600 grams of tissue was removed from the left side and 1,430 grams from the right side, resulting in a balanced and well-contoured chest appearance.

Kelly-Chest reconstruction, Double incision, Free nipple grafts

Before: The patient is 5 feet 2 inches tall and weighs 174 pounds, with a body mass index (BMI) of 32. On examination, he presents with moderately large breasts demonstrating bilateral Grade 3 ptosis. There is no evidence of a lateral chest roll. The patient desires a double-incision mastectomy with free nipple grafts to achieve a more masculine chest contour.

After: The patient demonstrates an excellent chest contour with well-defined masculine characteristics. The surgical scars are thin, flat, and non-pigmented. There are no dog-ear deformities bilaterally. The nipple grafts have healed well, though there is mild pigment irregularity noted on the right side.
A total of 805 grams of tissue was removed from the left side and 875 grams from the right side, resulting in a balanced and symmetrical outcome.
