Before: The patient is 5 feet 4 inches tall, weighs 120 pounds, and has a body mass index (BMI) of 22.3. On examination, he presents with a small chest without ptosis. He has previously been on puberty blockers, which limited breast development; however, he is dissatisfied with the small feminine contour and desires removal.
The patient is an excellent candidate for periareolar chest reconstruction. The surgical plan includes complete removal of associated breast tissue inferiorly and careful sculpting superiorly to achieve a masculine chest contour. Given the minimal fat present, the majority of the excision will involve breast gland tissue.

After: The right side exhibits some skin redundancy with minimal puckering. All incisions are well healed.
The chest demonstrates a masculine appearance with no residual breast tissue bilaterally, and healing is progressing appropriately.
A total of 105 grams of tissue was removed from the left side and 100 grams from the right side, resulting in a balanced and natural chest contour.

Casey, Keyhole chest reconstruction surgery

Before: The patient is 5 feet 2 inches tall, weighs 107 pounds, and has a body mass index (BMI) of 19.5. He presents with small breasts without ptosis, with the left breast slightly larger than the right. The nipple-areolar complexes (NACs) are located approximately 2 fingerbreadths below the pectoral split.
The patient exhibits good skin turgor. Breast tissue is noted to be moderately dense with some fluctuation, which will be considered in the surgical planning to optimize chest contour and symmetry.

After: The patient demonstrates good cosmetic results with a well-defined chest contour. Mild swelling is noted in the lower chest, slightly greater on the left side than the right.
A total of 105 grams of tissue was removed from the left side and 80 grams from the right side, contributing to a balanced and natural chest appearance.

Lucas,Keyhole, chest reconstruction surgery

Before: The patient is 5 feet 10 inches tall, weighs 135 pounds, and has a body mass index (BMI) of 19.4. On examination, he presents with a small chest and excellent skin elasticity. The nipple-areolar complexes (NACs) are appropriately sized and positioned, approximately 1 centimeter below the start of the pectoralis major.
Given these findings, the patient is an excellent candidate for a keyhole procedure, which can achieve a masculine chest contour while preserving nipple sensation.

After: On examination, the nipples are slightly larger than typical for males but maintain a natural appearance.
A total of 135 grams of tissue was removed from the left side and 115 grams from the right side, resulting in a balanced and aesthetically pleasing chest contour.

Mackennon, Keyhole, chest reconstruction surgery

Before: The patient is 5 feet 2 inches tall, weighs 110 pounds, and has a body mass index (BMI) of 20. He is lean, with small breasts and small nipples that are slightly low for a masculine chest but remain within an acceptable range. Examination reveals a fairly asymmetric bony chest wall.The primary consideration for a keyhole chest reconstruction is the potential for loose skin, which may require a revision procedure if the patient is dissatisfied with the outcome. Despite this, the patient’s nipples are appropriately sized and positioned, making him a suitable candidate for keyhole chest reconstruction. The patient is motivated to proceed with the procedure.

After: The procedure was completed successfully, and the patient demonstrates an excellent chest contour. The chest cavity is well filled, resulting in a natural and balanced appearance. A total of 120 grams of tissue was removed from the left side and 110 grams from the right side, contributing to the overall aesthetic result.
