Radical Breast Reduction, Double Incision, Free Nipple Graft, Lateral Chest Roll Excision

Tissue weight: L:2805g, R:2505g

Non-flat top surgery, radical reduction just reconstruction

Before: The patient is 5’6″ tall, weighs 181 pounds, and has a BMI of 29. On examination, they present with very large breasts. Given the degree of hypertrophy, a chest reconstruction with breast reduction using a free nipple graft is recommended. A Passot-type technique is believed to be an appropriate surgical approach to achieve optimal contour.

After: The patient underwent a nonbinary radical breast reduction to create an A/B cup breast size from a preoperative DD cup. An inferior pedicle technique was utilized to preserve the nipple–areolar complex, followed by a chest reconstruction approach to remove the remaining breast tissue and achieve a very small breast contour. A Wise pattern incision extending across the central chest was performed.

On examination, the incisions are healing appropriately. The scars appear mildly hyperpigmented but are fading and are expected to continue to improve over time. There is excellent symmetry and contour. A total of 1,150 grams of tissue was removed from the left breast and 1,230 grams from the right breast.

Before: On examination, the patient stands five feet eight inches tall and weighs 235 pounds, with a BMI of 35.7. The patient presents with large breasts bilaterally and Grade III ptosis.

After: On postoperative examination, the patient demonstrates a significantly improved breast contour with a more proportionate and balanced appearance. The breasts are well-shaped with improved positioning and symmetry following reduction of the significant breast volume and Grade III ptosis. A total of 1,265 grams of tissue was removed from the right breast and 1,205 grams from the left breast.

Non-flat top surgery, radical reduction just reconstruction

Before: On examination, the patient is relatively small in stature, standing five feet five inches tall and weighing 135 pounds, with a BMI of 22.5. Despite their slender frame, the patient has a significant amount of breast tissue with Grade II ptosis and an estimated C-cup breast size.

After: On postoperative examination, the patient demonstrates a significantly improved chest contour with a flatter, more proportionate appearance. The incisions are healing well, and the overall chest shape is smooth and symmetrical. The previous breast volume and Grade II ptosis have been successfully addressed, resulting in a more defined and aesthetically balanced chest contour.
